JOB TITLE:		Clinical Pharmacist
SALARY SCALE: 		Z10 - 6 ($58,338.42 - $65,261.50) Barbados Dollars

REPORTS TO:		Chief Pharmacist
DIRECT REPORTS:		Pharmacist II/Trainee Pharmacist

JOB SUMMARY
Under general supervision, provides direct patient-centered care and integrated pharmacy operational services with physicians, nurses, and other hospital personnel. Provides optimal clinical and pharmaceutical care activities in accordance with the department's mission, vision, and goals. Participates in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to patients.

MAJOR DUTIES AND RESPONSIBILITIES
The duties listed below are intended only as an illustration of the various types of work that may be performed:

1. Develops clinical pharmacy programmes according to policies and regulations;
2. Reviews records of patients to determine the appropriateness of medication therapy;
3. Identifies, designs, and implements improvements in the medication-use system to advance patient safety, maximize therapeutic outcomes, or control cost;
4. Provides accurate, safe, timely, and appropriate medication therapy based on patient age and needs;
5. Provides discharge medication review, reconciliation, and counselling as appropriate for patients;
6. Participates in structured ward rounds with medical teams;
7. Communicates effectively and appropriately with the multidisciplinary team and assures continuity of pharmaceutical care between shifts and among healthcare providers;
8. Facilitates pharmaceutical care services, direct patient care programs, medication utilization systems within assigned services and care areas to assure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards;
9. Tracks and evaluates assigned pharmacy programs for operational, quality, and financial efficiency and routinely benchmarks against national and regional best practices;
10. Completes critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug-related problems, improper drug or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failures;
11. Facilitates specialty medication procurement, ordering, and dispensing procedures including but not limited to chemotherapy, parenteral nutrition, controlled substances, etc. as appropriate for assignment;
12. Evaluate patient’s condition to ensure all issues are being treated;
13. Identifies high-risk patients and implements interventions to improve quality and safety;
14. Identify untreated health problems and refer patients to appropriate physicians;
15. Develop effective medication plans that minimize the risk of adverse side-effects
16. Consult on dosages, and substances;
17. Advise on the correct administration of drugs;
18. Assess the results of pharmaceutical treatments;
19. Collaborate with healthcare professionals to ensure optimal patient care;
20. Keep accurate documentation of medication plans and patient;
21. Serves as a member of the Drug and Therapeutic Committee;
22. Serves as a member of the Policy and Procedure Sub-Committee; and
23. Ensures compliance to quality, health, safety and environmental standards;
24. Using customer service skills, establishes and maintains effective working relationships with other employees, officials and members of the general public.
25. Performs any other related duty as assigned.

KNOWLEDGE, SKILLS & ABILITIES

1. Excellent knowledge of pharmaceutical therapy.
2. Knowledge of contemporary hospital/clinical practice and service.
3. Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicine in area appropriate for clinical assignments.
4. Familiarity with drug administration and health and safety procedure
5. Outstanding communication and interpersonal skills.
6. Strong customer service, problem solving, critical thinking and conflict resolution skills.

EDUCATION AND EXPERIENCE

1. Master of Arts in Clinical Pharmacy, Pharmacology or a similar relevant field.
2. Current professional registration as a Pharmacist.
3. Not less than five (5) years’ experience as a Pharmacist.
