Job Summary:          Internal Medicine will be part of a dynamic healthcare team focused on providing high-quality medical care to patients with a wide range of internal medicine conditions. The Senior House Officer will work under the supervision of experienced consultants to provide comprehensive medical services to patients admitted to our department and will participate in the assessment, diagnosis, treatment, and management of patients admitted to the internal medicine department. This role offers an excellent opportunity for professional growth and development in a dynamic healthcare environment.

Key Responsibilities:

Patient Care:

  • Conduct comprehensive medical assessments utilizing a systematic approach, including detailed history-taking, thorough physical examinations, and appropriate diagnostic investigations, to establish accurate diagnoses and formulate evidence-based treatment plans.
  • Prioritize patient care activities based on the severity of illness, clinical urgency, and resource availability, ensuring that patients with acute or life-threatening conditions receive timely and appropriate interventions.
  • Collaborate closely with consultants, specialists, and other members of the healthcare team to address complex medical issues, obtain expert input when needed, and coordinate multidisciplinary care for patients with multiple comorbidities or complex care needs.
  • Monitor patients' clinical progress through regular reassessment of symptoms, vital signs, and response to treatment, recognizing and promptly addressing any signs of deterioration or complications to prevent adverse outcomes.
  • Explain to each patient the outcome of any treatment or procedure once the care has been provided and ensure this is documented in the Medical Record.
  • Communicate the patient’s condition and treatment plan to the patient, patient’s family and collaborating physicians.
  • Ensure appropriate daily communication with the nursing team about the patient’s condition and plan of care.
  • Perform a daily bedside, verbal and written handover to the on-call physician prior to leaving the hospital compound.
  • Perform daily bedsides verbal and handwritten handover of all ICU, monitored and complex and acute cases to the Crash Doctor on call before leaving the hospital compound.

Patient Management:

  • Take a leadership role in coordinating the care of patients under your supervision, ensuring that care plans are implemented effectively, and that communication among healthcare team members is clear, timely, and accurate.
  • Foster therapeutic relationships with patients and their families through effective communication, active listening, and empathetic understanding of their concerns, values, and preferences.
  • Provide patient-centered care that respects cultural diversity, individual autonomy, and personal dignity, and promotes shared decision-making and informed consent.
  • Educate patients and families about their medical conditions, treatment options, and self-care strategies, empowering them to actively participate in their healthcare journey and achieve optimal health outcomes.
  • Advocate for patients' rights, needs, and preferences within the healthcare system, ensuring that their voices are heard and their interests are represented in care planning and decision-making processes

Medical Rounds:

  • Participate in medical rounds with attending physicians, consultants, and medical students to discuss patient cases, review treatment plans, and provide input on patient management strategies.

Diagnostic Evaluations:

  • Interpret diagnostic test results, including laboratory tests, imaging studies, and other specialized investigations, to inform clinical decision-making and guide the management of complex medical conditions.
  • Collaborate with interdisciplinary teams to coordinate diagnostic evaluations and treatment plans.

Treatment Planning:

  • Collaborate with the healthcare team to develop and implement comprehensive treatment plans for patients with internal medicine conditions, ensuring holistic and patient-centered care. This involves:
    • Clinical Assessment: Conduct thorough clinical assessments of patients, including reviewing medical history, performing physical examinations, and interpreting diagnostic tests, to formulate accurate diagnoses and identify patient care needs.
    • Evidence-Based Practice: Apply evidence-based medicine principles and clinical guidelines to guide treatment decisions, selecting appropriate pharmacological and non-pharmacological interventions based on patient-specific factors, disease severity, and treatment goals.
    • Medication Management: Prescribe and manage medications, adjusting dosages, frequencies, and formulations as needed to achieve therapeutic goals while minimizing adverse effects and drug interactions. Educate patients and caregivers on medication regimens, adherence, and potential side effects.
    • Therapeutic Interventions: Recommend and coordinate non-pharmacological interventions and therapeutic modalities, such as lifestyle modifications, dietary changes, physical therapy, occupational therapy, and counseling, to optimize patient outcomes and quality of life.Consultation and Referral: Consult with specialty services and multidisciplinary teams as needed to obtain expert input, guidance, or referrals for complex or specialized aspects of patient care. Facilitate seamless communication and coordination of care between healthcare providers and services.Care Coordination: Coordinate care transitions, discharge planning, and follow-up care arrangements to ensure continuity of care and seamless transitions between inpatient, outpatient, and community settings. Collaborate with case managers, social workers, and community resources to address patients' psychosocial needs and support services.
  • Monitor patient progress and response to treatment, adjusting plans as necessary under the guidance of senior medical staff.

Procedural Skills:

  • Demonstrate proficiency in performing a wide range of medical procedures commonly encountered in internal medicine, including but not limited to venipuncture, intravenous cannulation, arterial blood gas sampling, lumbar puncture, and nasogastric tube insertion.
  • Ensure the safe and effective performance of procedures by adhering to established protocols, using aseptic technique, and maintaining a high standard of infection control practices.
  • Continuously assess and reassess patients' procedural readiness, considering factors such as coexisting medical conditions, medication regimens, and psychological comfort, and providing appropriate pre-procedure preparation and post-procedure monitoring.
  • Recognize and respond promptly to procedural complications or adverse events, initiating appropriate interventions to mitigate risks and ensure patient safety.
  • Collaborate with consultants, senior medical staff, and other healthcare professionals in the planning and execution of more complex procedures, gaining valuable hands-on experience and expanding your procedural competency under supervision.
  • Perform procedures as required, ensuring all appropriate preparation and post procedure care is followed.
  • Develop and refine basic procedural skills relevant to internal medicine practice, including but not limited to:
    • Venipuncture: Perform venous blood draws for laboratory testing, ensuring patient comfort and safety.
    • Intravenous (IV) Line Placement: Insert peripheral IV catheters for administration of fluids, medications, and blood products, following aseptic technique and proper site selection.
    • Urinary Catheterization: Insert urinary catheters to monitor urinary output and relieve urinary retention, adhering to sterile technique and patient privacy.
    • Wound Care: Assess and dress wounds, incisions, and ulcers, applying appropriate wound care techniques and dressings to promote healing and prevent infection.
    • Nasogastric (NG) Tube Insertion: Insert NG tubes for enteral feeding, medication administration, or gastric decompression, ensuring proper placement and function.
    • Arterial Blood Gas (ABG) Sampling: Obtain arterial blood samples for analysis of oxygenation, ventilation, and acid-base status, following established protocols and safety guidelines.
    • Electrocardiogram (ECG) Acquisition: Perform basic ECG acquisition and interpretation to assess cardiac rhythm and conduction abnormalities, collaborating with senior staff for further evaluation.
    • Point-of-Care Testing: Perform point-of-care testing (e.g., glucose monitoring, urine analysis) using handheld devices to obtain rapid diagnostic results and guide patient management decisions.
    • Other Basic Procedures: Assist with other basic medical procedures and interventions as directed by senior medical staff, gaining hands-on experience and proficiency in clinical skills relevant to internal medicine practice.
  • Seek opportunities to learn and practice procedures under supervision.

Documentation:

  • Maintain meticulous and accurate medical records for all patients under your care, documenting pertinent clinical information, treatment interventions, and follow-up plans in accordance with institutional policies, regulatory requirements, and professional standards of practice.
  • Ensure that all medical documentation is completed in a timely manner and is easily accessible to members of the healthcare team involved in the patient's care, facilitating effective communication, collaboration, and decision-making.
  • Prepare death certificates with 48 hours, where postmortem results are not required.
  • Complete requests for Medical Reports within 72hrs.
  • Complete and submit incident reports for reportable events within 24hrs of occurrence.

Patient Education:

  • Explain to each patient the outcome of any treatment or procedure once the care has been provided and ensure this is documented in the Medical Record.
  • Provide patient education and counseling on disease management, medication adherence, lifestyle modifications, and preventive healthcare measures to promote patient understanding and engagement in their care.

Collaboration:

  • Collaborate effectively with interdisciplinary healthcare team members, including nurses, pharmacists, social workers, and allied health professionals, to ensure comprehensive and coordinated patient care.
  • Provide teaching, oversight and supervision of medical students and interns in the care and treatment of the patient and in the performance of required procedures. 

Professional Development:

  • Participate in educational activities, conferences, and continuing medical education (CME) opportunities to enhance clinical knowledge, skills, and competency in internal medicine practice.

Qualifications:

  • Medical degree (MBBS or equivalent) from a recognized medical school.
  • Registered Medical Practitioner with the St. Lucia Medical & Dental Council to practice in St. Lucia.
  • Successful completion of two years rotating or working experience in one of the clinical disciplines which includes Surgery, Obstetrics and Gynecology, Internal Medicine and Pediatrics.
  • Strong interest in internal medicine and commitment to pursuing a career in this specialty.
  • Advanced cardiac life support (ACLS) certification.
  • It is desirable to have successful completion of CAMC part I and II or an equivalent examination.

Working Conditions:

  • Work Location: Hospital-based position in the Internal Medicine Department.
  • Schedule: Full-time position
  • Hours: Rotating shifts, including days, evenings, nights, weekends, and holidays, as required by departmental schedules.
  • Environment: Fast-paced clinical environment with exposure to patients with acute and chronic medical conditions. May involve exposure to infectious diseases and hazardous materials.
  • Equipment: Use of medical equipment and technology for patient evaluation, diagnosis, and treatment.
  • Physical Requirements: Standing, walking, lifting, and performing procedures as required in a clinical setting.
  • Restrictions:
  • A Consultant Physician will customarily see any complex or seriously ill patient. The Senior House Officer must Immediately call-out a consultant physician in complex or acute cases.
  • Be available as is required when on call. Respond within 5 minutes to all emergencies. Promptly respond to calls with appropriate evaluation and action within 20 minutes.
