The Ministry of Health and Human Services is seeking a qualified Consultant to conduct an assessment on the potential establishment of a Health Services Authority (HSA) in the Turks and Caicos Islands.

Scope:
The Consultant will review existing health sector structures, conduct stakeholder consultations, assess institutional, legal, and financial readiness, and develop an implementation roadmap. Deliverables include an inception report, draft assessment report, stakeholder validation workshop, and final report.

Qualifications:

• Master’s degree or higher in Public Health, Health Administration, Health Policy, Business Administration, or related field.
• Minimum 10 years’ experience in health systems development or institutional reform.
• Proven experience with HSA assessments or similar work, preferably in the Caribbean.
• Strong analytical and report-writing skills.

Duration: 12 weeks

Terms of Reference (TOR) For a Consultant to Conduct the Assessment for Implementation of a Health Services Authority (HSA-Exploratory Work) in the Turks and Caicos Islands

1. Background Context

The Turks and Caicos Islands (TCI) have experienced significant growth in their healthcare needs, driven by an increasing population and a growing demand for quality medical services. To address these challenges, the government is considering the establishment of a Health Services Authority (HSA) to enhance healthcare delivery, management, and sustainability.

The development of an HSA would enable TCI to streamline healthcare operations, improve patient care, and integrate financial mechanisms to ensure long-term viability. Drawing from successful models in neighboring regions, such as the Cayman Islands, Bahamas and the British Virgin Islands (BVI), which have both established a robust Health Services Authority, TCI aims to create a similar framework tailored to its unique socio-economic and regulatory environment.

This consultancy will conduct an exploratory assessment to determine the feasibility, readiness, and strategic pathway for establishing an HSA in the Turks and Caicos Islands. The assessment will identify institutional, legal, operational, and financial considerations and propose actionable recommendations for phased implementation.

2. Objective

The objective of this consultancy is to conduct an assessment to inform the potential establishment of a Health Services Authority (HSA) in the Turks and Caicos Islands. The consultant will review existing health sector structures, assess feasibility, and develop a roadmap outlining implementation options, associated costs, and anticipated benefits

 3. Scope of Work

Under the guidance of the Health Policy and Planning Unit, the Consultant will carry out the following tasks: 1. Conduct a literature review of existing health sector policies, laws, organizational structures, and institutional arrangements.

2. Undertake stakeholder interviews with relevant government departments, development partners, health professionals, and other key actors.

3. Conduct an institutional analysis to assess current roles, mandates, and coordination mechanisms in health service delivery.

4. Undertake a legal and regulatory review to determine the enabling or constraining factors for establishing an HSA, including potential legislative or policy amendments.

5. Perform a financial analysis to assess funding flows, budgeting processes, and financial sustainability options for the proposed Authority.

6. Develop options for institutional design, including governance, organizational structure, and accountability mechanisms for the HSA.

7. Prepare a draft report presenting key findings, proposed structure, and implementation roadmap.

8. Facilitate stakeholder validation workshops to present and refine proposed recommendations.

9. Submit a final report incorporating feedback and endorsed recommendations for the establishment of the Health Services Authority.

4. Qualifications of the Consultant The consultant must possess the following qualifications and experience: • Education: Advanced degree (Master’s or higher) in Public Health, Health Administration, Health Policy, Business Administration, or a related field.

• Experience: At least 10 years of experience in health systems development, reform, or institutional strengthening, with proven experience in HSA establishment or assessment.

• Skills and Competencies: Strong analytical, research, and strategic planning skills; proven ability to engage stakeholders and facilitate consultations; excellent report writing and presentation abilities.

• References: At least two professional references from previous relevant projects, preferably including experience with Caribbean health authorities.

 5. Roles and Responsibilities Consultant:

• Conduct literature review and stakeholder interviews.

• Undertake institutional, legal, and financial analyses related to the establishment of the HSA.

• Develop and present draft and final assessment and design reports, including proposed governance structures and implementation roadmap. Project Lead:

• Provide access to relevant documents, data, and contacts.

• Facilitate stakeholder engagement, interviews, and review sessions.

• Provide technical oversight, guidance, and quality assurance throughout the consultancy process. Stakeholders:

• Participate in consultations, interviews, and validation workshops.

• Provide feedback, contextual information, and insights to ensure the accuracy and relevance of the proposed recommendations.

5. Deliverables and Timelines

The consultancy will be conducted over 12 weeks, with deliverables aligned to specific phases as outlined below:

Weeks 1–2 Initial meetings, review of background data, work plan development, stakeholder mapping. Inception Report – detailing methodology, work plan, and data collection approach.

Weeks 3–6 Data collection and stakeholder consultations. Ongoing consultation summaries and data analysis updates (internal progress notes).

Weeks 7–9 Analysis of findings, institutional and legal assessments, and initial report drafting. Draft Assessment and Design Report – summarizing findings, institutional design options, and implementation roadmap.

Week 10 Stakeholder review and validation workshop. Stakeholder Validation Workshop – presentation and discussion of draft findings and recommendations.

Weeks 11–12 Incorporation of feedback, finalization, and submission of report. Final Report – integrating stakeholder feedback and providing a finalized institutional, legal, and financial proposal   

 7. Proposed Consultancy Fee

The consultant’s remuneration will be based on experience, qualifications, and the scope of work.

8. Quality Assurance and Review

All deliverables will be reviewed by the Project Lead and a designated review committee to ensure that outputs meet quality standards, address the TOR objectives, and align with policy priorities.

10. Completion and Approval

The consultancy will be considered complete upon submission and approval of the final report by the project lead and endorsed by the Ministry of Health and Human Services
