CLINICAL PHARMACIST
PRIMARY DUTIES & RESPONSIBILITIES (included but not limited to):
Attends and participates in Patient Care Rounds with physicians and nurses to evaluate patient progress.
Identifies, assesses and solves medication-related problems and provides a clinical judgment as to the continuing effectiveness of individualized therapeutic plans and intended therapeutic outcomes.
Conducts visual physical assessments evaluates patient problems, and monitors medications and/or laboratory tests in accordance with established standards of practice.
Provides a ward pharmacy service by checking the drug charts of all patients in order to clarify any ambiguities, to initiate supply of those drugs not stocked o the wards and to intervene in the prescribing, supply, or administration processes whenever necessary.
Monitors, evaluates and recommends modifications in drug therapy to ensure safe and effective patients care.
Prepares weekly reports on clinical matters of the BVIHSA which will be discussed with the Pharmacy Manager for possible solutions.
Provides an on-call pharmacy service during off duty hours in the absence of the Pharmacy Manager.
Retrieves, evaluates, utilizes and manages data and professional resources for the purposes of information sharing with other health professionals and to enhance patients care. Serves as a drug information resource for physicians and nurses on the wards.
Collaborates with clinicians, pharmacists and other health care professionals (through various committees) in the development of formularies, treatment protocols and the review and evaluation of drug use.
Dispenses medications and counsels outpatients in the dispensary as necessary.
Documents and tracks medication errors and implement clinical intervention and submit information to Pharmacy and Therapeutics Committee.
Performs other duties as assigned by the Pharmacy Manager.
Minimum Requirements:
Education & Experience
Bachelor’s Degree in Pharmacy and three years’ clinical experience OR
Master Degree in Clinical Pharmacy OR
Doctor of Pharmacy Degree
Three years’ experience as Clinical Pharmacist in a health care facility
Licenses/Certifications
Must be registered with the Allied Health Council
Acquire a minimum of 9 CE credits annually
Knowledge
Expert knowledge of advanced therapeutics and pharmacokinetics
Profound understanding of pharmaceutical care concept and practice
Proficiency in general computer applications
